Follow these steps on how you can download your vaccination card into your iPhone. This can help when it comes to show proof of your vaccines when you go to gym, restaurants, and etc. Below are the steps to download your digital card into Apple Wallet.

STEPS TO OBTAIN DIGITAL VAX CARD

1. Go to IL Dept Public Health portal: idphportal.illinois.gov

2. Click on create account.

3. After creating your account, click on smart health card tab.

4. Scan QR code and your vaccination card will download to Apple Wallet.
