At Provida Family Medicine, we aim to make every visit as comfortable and uncomplicated as possible. We are happy to answer any questions you may have about treatments, check-ups, or any other information you’d like to know about your health or the health of your family.
To that end, here are some frequently asked questions regarding visits to our Gurnee office.
How do I book an appointment?
Patients can book an appointment with Provida Family Medicine 24/7 through the Healow App and anytime on our website.
Healow & Patient Portal: With Healow, gain easy, private access to your medical information online whenever and wherever you have internet access. All we need is your email address to get you started, or download the Healow App and enter code BIGDAA to sign up.
Take charge of your health, log onto your portal frequently!
You may also call 847-548-2200during normal office hours to request an appointment with our front desk staff. We offer same-day, early morning, late evening, and Saturday appointments.
How do I confirm an appointment?
Provida Family Medicine uses an automated confirmation system to confirm appointments with patients.
When we call to confirm a future appointment, please listen to the entire message, then PRESS 1 to confirm, PRESS 2 to cancel, or PRESS 3 to change your appointment to another date and time.
Please notify us of appointment changes by NOON the day before the appointment at the very latest.
What is your walk-in policy?
Walk-in appointments are always welcome!
If you would like to see a doctor without an appointment, patients are encouraged to come to our Gurnee, IL, office during regular business hours:
Monday – Friday 8am – 6pm
Saturday 8am – noon
Sunday Closed
If you or a loved one is having a medical emergency, please call 9-1-1. Patients can contact Provida Family Medicine after the emergency so that we can follow up with your care center for information about our patients.
What should I bring to an appointment?
Patients should make sure to bring the following to their appointment:
- Photo Identification
- Insurance Card
- Co-Payment or Co-Insurance
- Bottles of all medication you are taking
- Medical immunization records
- Information from other medical facilities
- Your health questions and concerns
Please arrive 15 minutes before each appointment to allow time for registration.
What insurance is accepted?
We accept most major insurance plans including HMOs and Medicare. We also have special pricing for our patients that do not have insurance. We provide equal access to all patients regardless of insurance status.

How do I access my medical records?
Patients can request their medical records by filling out a form in the Patient Portal.
After this form is filled out, it will be scanned in a patient’s chart for our Medical Records Coordinator to review for accuracy.
The form will then be forwarded to MediCopy. The turnaround time to receive your records after the form is filled out is 1-2 weeks. Patients can direct all questions to MediCopy at 866-587-6274.
If the records are bring requested from another doctor, we follow steps 1-2, and then the Medical Records Coordinator will release the records to the party requesting them.
How do I contact Quest Labs?
Your Provida Family Medicine provider may order lab tests and analyses. Quest is our preferred laboratory and is conveniently located on-site. Results are readily posted via our online Patient Portal. However, if you have questions or concerns we encourage you to call our office directly at 847-548-2200 to discuss them with your provider.

What are your hospital affiliations?
Provida Family Medicine is affiliated with Condell and has portal access to Lake Forest Hospital and Vista Medical Center.
What are your disability insurance policies?
Patients must bring their disability insurance form to the appointment and must fill out the patient portion of the form.
Providers will fill out the Physician part of the form and then will forward the form to MA. MA will then fax the form to the destination number listed on the form.
We do not call for the patient if they have questions unless instructed by the doctor.
How can I pay a bill?
Payment for your visit is made during the check-in process before your appointment. We accept cash, check, debit, and several major credit cards.
Patients can also pay their bills via the USPay Patient Portal.
How can I receive notes about my appointment for school or work?
For patients seeking an appointment because of work or school, they will receive a note from their doctor as proof of visit.
This is taken care of at the time of the visit with the provider, but patients must be evaluated to receive a note unless otherwise instructed by a doctor.
